I've read that others have used the VRSF without issue. I think you toyed with longer bolts, correct? Would you think this contributed to the gap/failure?

Yep, use shorter bolts that are threaded all the way up. Plus make sure they are welded in straight... needless to say, mines weren't. Here's a pic for ref.
