horrible rattling noise high revs

Hi, driving home tonight and engine starts making a rattling noise , loud if reving car in stationary position, can’t really hear it on tick over, until about 1500 revs or more and it’s sometime like a grinding noise too, and my engine cut out once , then it had trouble starting it again , but did start
This is the code I got 004596 smooth running controller cylinder 3,also the other code is 00cf60 is this the cam chain failing? Bmw x5 40d e70 2011 140k

I’ve cleared code and it still does it , sound like something is grinding on start up sounds horrible, started but hesitated

I have a 4.0dx 2010 with 191k not sure in miles what that is but it is somewhere around yours. You will need to do the rocker cover gasket and the oil, cooling heat exchanger gasket. Easier to explain, where you remove the oil filter. That whole assembly needs to be removed and the gasket replaced. The leak you have now is coming from the rocker cover gasket but it will not be long before the other one goes. The noise you have does not sound metallic more of a squeak, however it could be a chain on a plastic guide. You need to be logical. What spins when the engine is idling? Crank, pistons, drive belt, water pump, ac compressor, steering pump, camshafts, timing belt, high pressure fuel pump, oil pump etc. Do not forget the oil pump is also the vacuum pump providing vacuum for your brakes, engine mounts, turbos etc. Is it a vacuum leak? You need to sit back and be logical.
Decisions need to be made. As mentioned before to do the gaskets you need to do, the intake manifold needs to be removed. If you do that then you need a walnut blast on your valves, intake manifold, egr, etc all need to be cleaned and while that is done why would you not change your glow plugs and controller while the intake manifold is off. You can see where I am going here, How far do I go? I can tell you I just did my rocker cover gasket and injector 6 snapped and I was in a world of hurt getting it out.
If you want to keep the car for another 145,000 miles all these things need to be done, even the timing chain, it is due to be replaced.
